The Challenge
Motor Neuron Disease (MND) is a progressive, life-limiting neurological condition requiring rapidly evolving, high-intensity supports.
At service commencement, the participant required moderate assistance with mobility, personal care, and fatigue management.
Over time, their needs escalated significantly to include:
- Loss of mobility and transition to powered wheelchair
- Swallowing impairment requiring PEG feeding
- Respiratory decline requiring CPAP support
- Increasing dependency for all Activities of Daily Living
This required a service model capable of anticipating progression, managing clinical risk, and scaling support safely and efficiently.
Our Approach
1. Proactive, Future-Focused Planning
From the outset, VIVA Mutual implemented a forward-planning model, anticipating disease progression and aligning supports early.
2. Nurse-Led Clinical Governance
A Registered Nurse led care delivery through:
- Comprehensive clinical assessments
- Detailed care plans and risk protocols
- Delegation of high intensity supports (PEG feeding, CPAP)
- Ongoing competency-based training and supervision
3. Workforce Capability and Scaling
Support workers received specialised training in:
- High intensity supports (enteral feeding, respiratory care)
- Manual handling and 2:1 transfers
- Fatigue-aware care practices
Staffing models evolved from 1:1 to 2:1 as complexity increased.
4. Integrated, Multidisciplinary Care
Close collaboration with:
- Support Coordinator
- MND SA (specialist education and guidance)
- Hospital and acute care teams
Ensured continuity across home, hospital, and end-of-life care.
5. Evidence-Based Funding Advocacy
Detailed clinical documentation supported successful NDIS funding reviews, ensuring appropriate resourcing for increasing care needs.
