The Challenge:
CI, aged 65, was living with early onset dementia, Alzheimer’s disease, and significant post-stroke complications. Living alone with 24/7 support, her condition continued to decline, with:
- High falls risk and physical deterioration
- Cognitive decline, confusion, and refusal of care
- Social isolation and withdrawal
- Full dependence for daily living
A new model of care was required to ensure safety, sustainability, and improved quality of life.
